Keep a concise record of symptoms, decisions and recovery checks during website incidents. Avoid filling logs and support messages with private data.

  2. Keep a change timeline
  3. Use the record during handoff
  4. Close with practical follow-up

When several people investigate an outage, useful details can disappear into separate chats. An incident record helps the next person understand what happened and what has already been tried.

Start with observable facts

Record the affected URLs, time, visitor state and safe error reference. Separate observations from hypotheses. “The form returned an error at this time” is different from declaring a particular plugin responsible without evidence.

Keep a change timeline

Note relevant configuration changes, approvals and recovery actions. Record who performed them and what was verified afterwards. Avoid publishing passwords, tokens, complete customer messages or payment credentials in the incident record.

  • Preserve non-sensitive examples of the failure.
  • Record the last known working state.
  • Identify affected business processes.
  • Keep decisions and remaining questions visible.
  • Link the final checks and follow-up owner.

Use the record during handoff

If an investigation moves from the developer to a hosting provider, a concise timeline can prevent repeated experiments. Share only what the recipient needs and is authorised to access. Do not post private log archives publicly to make them easier to download.

Close with practical follow-up

Document the confirmed cause where established, the recovery and any uncertainty. A suspected explanation should remain labelled as such. The strongest record helps prevent repetition and improves the response plan without inventing certainty about every technical detail.
